Retiree Life Insurance Beneficiary

You designate who you want to receive the proceeds from your Retiree Life Insurance by completing a beneficiary form. Beneficiary forms are currently maintained in the NU Benefits department.

To change your beneficiary, print out a copy of the Prudential's Life Beneficiary Form, (PDF: 324KB) complete it and mail it to:

You can change your beneficiary at any time by completing a new Beneficiary Form and returning it to Prudential.
