Change in Marital Status

Please notify an NU Benefits Representative if your marital status changes. Below are some important considerations when you change your home address.

Health Benefits
If you marry, your spouse becomes an eligible dependent and can be added to the health plan.

If you divorce, your former spouse no longer qualifies as an eligible dependent. Your spouse will be eligible to continue coverage under COBRA continuation for a limited period of time by paying the full cost of the coverage (no company contributions).

Beneficiaries
Beneficiaries are the people you name to receive the proceeds from your Retiree Life Insurance and 401k Plan. Beneficiary cards are currently maintained in the NU Benefits department.

Retiree Life Insurance Print out a copy of the Prudential's Life Beneficiary Form, (PDF: 324KB)
complete it and mail it to:

401k If you are married, your spouse is automatically your beneficiary unless
s/he agrees in writing to your naming someone else. Contact an
NU Benefits Representative for a 401k Beneficiary Card.

Tax Withholding
You may want to consider changing your withholding taxes on your pension payments to reflect the change in your marital status. Complete the appropriate tax withholding forms below and mail them directly to Aetna:

  • Federal Tax Withholding form W4P (PDF: 164KB)
  • State Tax form, if you live in a state with a state income tax. Links to the forms for CT and MA are identified below.
