You can order new or replacement ID cards directly from the carriers either on-line or by telephone. Follow this link for contact information.
Note:
There are no ID cards for the following benefit programs:
| Dental |
Simply tell your dentist that you are
covered through Northeast Utilities, Delta Dental of
New Jersey, Group Number 5480. |

| Vision |
If you retired prior to 1991 (or 1993 from Public Service
of New Hampshire), you are covered for vision benefits through
Vision Services Plan (VSP). When you visit a provider
who participates in the VSP network, simply provide your Social
Security Number and s/he can confirm your eligibility. When
you receive care from a provider who does not participate in VSP,
you pay the full cost and submit the receipt to VSP for partial
reimbursement. |
