Beneficiary Change

Beneficiaries are the people you name to receive the proceeds from your Retiree Life Insurance and 401k Plan. Beneficiary cards are currently maintained in the NU Benefits department.

Retiree Life Insurance	To change your beneficiary, print out a copy of the Prudential's Life Beneficiary Form, (PDF: 324KB) complete it and mail it to:

401k	If you are married, your spouse is automatically your beneficiary unless s/he agrees in writing to your naming someone else. Contact an NU Benefits Representative for a 401k Beneficiary Card.
