Change Your Address

It's important that we have your current address in our company databases so that important home mailings reach you on a timely basis. To change your address, complete a Personal Information Change form and send it to an NU Benefits Representative at the address listed on the form.

Below are some important considerations when you change your home address.

Health Benefits
If you move out of your medical carrier's service area, or if as a result of your move there is a significant change in your premiums, you can change your medical option. If your move is temporary, such as spending the winter in Florida, you may be eligible for guest privileges if offered by your carrier. To learn more about guest privileges, contact your carrier's Member Services department at the telephone number listed on your medical ID card.
